How it works
Four simple steps before your appointment.
Step 1
Download
Choose the form you need below and save the PDF to your computer.
Step 2
Fill in on your computer
Open it in Adobe Acrobat Reader (free) and type your details into the fields.
Step 3
Save, print and sign
Save your completed form, print it and sign it by hand in blue or black pen.
Step 4
Bring it with you
Bring the signed form and all required documents to your appointment at the Embassy.

Passports
Form A1
Lebanese passport application form. Required with every passport application.
Form A2
Additional passport form. Bring it completed if it applies to your application.
Consent form
Consent form for applications that require the consent of a parent or guardian.
Visas, citizenship and fees
Tips for filling in your form
- Use a computer, not a phone. Phone previews often cannot save typed text.
- Open the PDF in Adobe Acrobat Reader (free), Chrome or Microsoft Edge.
- Write names exactly as they appear in your Lebanese documents.
- Choose Save after typing, then print the saved file.
- Sign by hand. Typed or digital signatures are not accepted.
Bring the original signed form
Forms are not accepted by email. Bring the printed, signed form with your original documents to your appointment.
